Monica Polk, DO
Dr. Polk is a Board Certified Adult, Adolescent and Child Psychiatrist and Board Eligible Forensic Psychiatry. Dr. Polk received her professional training from University of New England College of Osteopathic Medicine. She began as an Intern at Hartford Hospital/Institute of Living and completed her Residency in Adult Psychology at Thomas Jefferson University Hospital. She then went on to complete her Fellowship in Child and Adolescent Psychiatry at George Washington University and a Fellowship in Forensic Psychiatry at the University of Maryland Medical Center. Dr. Polk was a 1st Lieutenant / Administrative Officer in US Army Reserve Medical Service Corps.
Lane Smith, MD
Dr. Smith brings a wealth of experience and expertise to Salt Lake Behavioral Health. He is a Distinguished Life Fellow of the American Psychiatric Association; Board Certified in Psychiatry; a Past President of the Utah Psychiatric Association; and a former Clinical Assistant in both the Departments of Psychiatry and Family Practice at the University Of Utah School Of Medicine. He has also been a Medical Director at two facilities and has given lectures to professionals as far away as Nova Scotia. Dr. Smith is a thorough diagnostician who takes the time to carefully educate his patients. Among many duties while serving as a medical officer in the USAF, he was a Deputy Commander of both an Air Force Dispensary and Hospital. We are pleased to have him working with our other fine staff at Salt Lake Behavioral Health.
Gerald Lazar, MD
Dr. Lazar obtained his undergraduate degree from Cornell University in Ithaca, New York and his Medical Degree from Albany Medical College. His training included a postgraduate internship at Albany Medical Center and a residency at Johns Hopkins Hospital in Baltimore. Dr. Lazar is well-respected in the Salt Lake community and has held many direct care and Medical Directorship positions throughout his extensive career.
Sean Ponce, MD
Dr. Ponce attended Brigham Young University and went on to medical school at the Oregon Health Sciences University in Portland. He completed one year of residency in Spokane, Washington and then completed a Family Medicine residency in Salt Lake City. Dr. Ponce specializes in the treatment of addiction and is particularly skilled with patients who are dually diagnosed with both mental health and addictive issues. He is a past President of the Utah State Chapter of the American Society of Addiction Medicine. He is Board Certified by the American Board of Family Medicine and the American Board of Addiction Medicine.
Brad Workman, APRN
Brad is a licensed Advanced Practice Nurse Practitioner and a board certified Psychiatric Mental Health Nurse Practitioner. Brad’s training includes Bachelor of Science and Master of Science degrees, both in nursing, at Brigham Young University. He completed his post-graduate work at the University of Utah. Brad has a wide range of experience and expertise in the assessment and treatment of both mental health and substance abuse disorders in both adults and older adults.
France Davis, PA
France is a licensed Physician Assistant who obtained his undergraduate degree from Georgetown University in Washington, D.C. and his Master of Science in Physician Assistant Studies from Marywood University in Scranton, Pennsylvania. France has a wide range of experience and has had extensive training as a neuroscience clinical drug trials Sub-Investigator and Psychometric Rater. His expertise is in the treatment of both mental health and substance abuse disorders.
Jean Stanford, PA
Jean is a board certified Physician Assistant. She received her RRT at Weber State University. She then went on to the University of Utah, completed the Physician Assistant Program and has been a licensed PA since 1998. Jean has a wide range of experience including mental health and substance abuse treatment with both adolescents and adults of all ages.
